Effectiveness and safety of a flexible family visitation model in adult intensive care units: a cluster-randomized, crossover trial (The ICU visits study).

Abstract
Objective: To compare the effectiveness and safety of an flexible family visitation model (FFVM) versus a restrictive family visitation model (RFVM) on delirium prevention among ICU patients, as well as to analyze its potential effects on family members and ICU professionals. Methods: A cluster-randomized crossover trial which compares a FFVM (12 h/day) with a RFVM (<4.5 h/day), was conducted in 40 mixed adult ICUs in Brazil. Participant ICUs were randomly assigned to 1 of 2 sequences of interventions (FFVM followed by RFVM or RFVM followed by FFVM) in a 1:1 ratio. The primary outcome was the cumulative incidence of delirium measured using the Confusion Assessment Method for the ICU. Secondary outcomes included daily hazard of delirium, ventilator-free days, any ICU-acquired infections, ICU stay, and hospital mortality among the patients; symptoms of anxiety and depression and satisfaction among the family members; and prevalence of burnout syndrome among the ICU professionals. Tertiary outcomes included relevant measures of effectiveness and safety of ICU visiting policies among patients, family members and ICU professionals. Results: In total, 1688 patients, 1270 family members and 829 ICU professionals were included in the study. The results of the effects of FFVM on patients, family members and ICU professionals will be presented in the 2018 CBMI congress. Conclusion: The results of the ICU Visits study will allow health care professionals, researchers, and policymakers to drawn conclusions about the effectiveness and safety of a flexible family visitation model for delirium prevention in adult ICUs.
