Abstract
The article discusses the advancements in the treatment of type 1 diabetes (T1DM) through closed loop insulin delivery systems. These systems involve continuous delivery of insulin via an infusion pump and real-time glucose monitoring devices. While these advancements have improved metabolic control, their impact on glycemic outcomes has been disappointing. However, the hybrid closed loop (HCL) automated insulin delivery system has shown promising results, with patients spending a significant amount of time within the desired glycemic range. There are ongoing efforts to develop patient-tailored algorithms using artificial intelligence and machine learning to further improve glycemic control. The main limitation of HCLs is the subcutaneous delivery of insulin, which does not match the physiological profile of insulin concentrations in the bloodstream. The article concludes that while HCLs represent a game-changing advancement in T1DM care, further optimization is needed to overcome technical issues related to insulin and its delivery.
