Abstract
This article discusses a case study of a man in his 30s with acute promyelocytic leukemia (APL) who was treated with arsenic trioxide and experienced recurrent loss of consciousness. The patient's electrocardiogram (ECG) showed a prolonged QTc interval and a distinct beat-to-beat alternating of opposite T-wave polarity, known as macrovolt T-wave alternans (TWA). The patient later developed torsades de pointes (TdP), a life-threatening ventricular arrhythmia. The article concludes that the treatment of relapsed APL with arsenic trioxide can prolong the QTc interval and increase the spacial heterogeneity of repolarization, leading to the occurrence of ventricular tachyarrhythmias.
