Distinct Plasma Extracellular Vesicle Transcriptomes in Acute Decompensated Heart Failure Subtypes: A Liquid Biopsy Approach.

Abstract
This research letter discusses a study on the use of liquid biopsy, specifically plasma extracellular vesicles (EVs), to analyze the transcriptomes of different subtypes of acute decompensated heart failure (HF). The study aimed to identify distinct transcriptomes between HF with preserved ejection fraction (HFpEF) and HF with reduced ejection fraction (HFrEF). The researchers found that specific plasma transcripts effectively distinguished between HF subtypes, but other EV transcripts were commonly dysregulated in both subtypes. The study suggests that EVs could be used as a "liquid biopsy" for cardiovascular diseases, but more specific markers are needed to assess HF status and multiorgan function. Another study discussed in this document used RNA sequencing to analyze circulating extracellular RNAs (exRNAs) in plasma samples from patients with HFpEF and HFrEF. The results showed that the transcriptomic profiles of exRNAs differed between the two subtypes, with HFrEF showing a signature centered on cardiomyocyte origin and HFpEF showing a broader, systemic disease profile.
