Abstract
The article discusses a study that describes the increased involvement of pharmacists in collaborative care across a not-for-profit, community health system to design patient care in a novel synchronized visit model. Highlights include structure for synchronized visits between pharmacists and primary care providers, integration of the visits into the physician's existing full schedule and workflow, and the interprofessional efforts needed to successfully deploy a synced visit model.
