Colonic interposition graft and risk of adenomatous polyp. Do we need screening?

Abstract
This article discusses the development of polyps and colonic carcinomas within grafts used for esophageal reconstruction, specifically focusing on a case study of a 58-year-old male who underwent total esophagectomy and subsequent colonic interposition 38 years ago. The patient developed severe pulmonary fibrosis due to chronic colonic graft reflux, leading to the need for a percutaneous endoscopic jejunostomy tube insertion. During this procedure, two polyps were discovered in the colonic graft. The initial endoscopy revealed two polyps, and subsequent endoscopies showed an increase in size. Polypectomy was eventually performed, and histopathologic evaluation revealed tubular adenoma with high-grade dysplasia and tubular adenoma with low-grade dysplasia. The article concludes that colon cancer screening of grafts is recommended for this group of patients, and the importance of surveillance should not be underestimated. [Extracted from the article]
