Predicting the need for step-up after EUS-guided drainage of peripancreatic fluid collections, including QNI score validation: a prospective cohort study.

Abstract
This article, published in the journal Endoscopy, explores factors that predict the need for additional procedures after EUS-guided drainage of peripancreatic fluid collections (PFCs). The study included patients who underwent drainage with either Lumen Apposing Metal Stents (LAMS) or Double-pigtail Plastic Stents (DPPS) between 2020-2023. The researchers found that patients who required a step-up to additional procedures had a higher number of quadrants involved, a more frequent paracolic extension, a higher necrosis content, and were in the high-risk group according to the Quadrant / Necrosis / Infection (QNI) score. The QNI score was the only independent predictor of the need for additional procedures. The study suggests that the QNI score may have utility in guiding the choice between LAMS and DPPS.
