Abstract
Brain computed tomography (CT) and magnetic resonance imaging (MRI) without contrast administration did not show significant abnormalities. Temporal artery biopsy (TAB) then revealed inflammatory cells infiltrating the temporal artery wall, with the presence of lymphocytes, plasma cells, and giant cells; inner elastic lamina was fragmentated. The persistence of mydriasis in both eyes and the absent response to light with stimulation in both eyes after good visual acuity recovery in left eye confirmed that bilateral mydriasis was not secondary to the severe visual loss.
