Abstract
The article discusses Kobe dementia model' is a check-up system of dementia in Kobe City, Japan, initiated from January 2019. Topics include the identification of subjects who have at risk for dementia has an important issue; and the DASC-21 has a list of 21 questions about impairment of cognitive functions and functioning in daily life.
