Abstract
The article introduces a new approach for patients with long term conditions (LTCs). It mentions about Care and support planning (CSP) as a new way of providing routine care in general practice for people living with single or multiple LTCs or complex situations such as frailty; and also highlights the use of provider or professional language by practice teams.
