Abstract
This letter to the editor discusses the case of a 76-year-old patient who was evaluated for a possible lymphoid malignancy. The patient had a history of gastrointestinal stromal tumor (GIST) and was treated with Imatinib, which led to complete remission. Further examination revealed the presence of a B-cell clone in the peripheral blood, but over a period of 5 years, there was no progression to chronic lymphocytic leukemia (CLL). Molecular analysis confirmed the presence of a BIRC3 mutation, which is associated with CLL but not typically seen in monoclonal B lymphocytosis (MBL). This case highlights the complexity of CLL evolution and represents the first reported case of BIRC3-mutated low-count MBL with long-term stability. [Extracted from the article]
