Abstract
The article discusses the concept of lung recruitment in acute respiratory distress syndrome (ARDS) and its implications for positive end-expiratory pressure (PEEP) settings. Lung recruitment refers to the process of reaerating atelectatic units and improving the inflation status of poorly aerated units. The article explains the forces involved in opening atelectatic units and emphasizes the importance of maintaining recruitment through adequate PEEP levels and tidal volumes. Different methods for assessing recruitment, such as gas exchange-based, CT-scan-based, and gas-volume-based methods, are also discussed. The article concludes that while recruitability provides important information about disease severity, PEEP selection should consider other variables such as elastance, transpulmonary pressures, and hemodynamics for personalized ventilation settings.
