Abstract
The article presents the discussion on accurate documentation in clinical medicine being vital for delivering safe patient care. Topics include litigation occuring less frequently in anaesthesia than other specialities, medicolegal claims relating to obstetric anaesthesia increasing in South Africa; and accidental awareness under general anaesthesia as the leading cause of successful litigation against obstetric anaesthetists.
