Abstract
Introduction: Deformities of the middle ear ossicles without malformations in either the outer or inner ear characterize isolated congenital ossicular chain malformations with the symptom of hearing loss, an alteration that may go unnoticed by the patient and not be correctly diagnosed by the doctor. Objectives: To report a case of ossicular chain malformation and the results of an ossiculoplasty with an incus autologous prosthesis Resumed report: Male, 42 years old, with a history of hearing loss bilaterally since childhood, with insidious onset and progressive worsening over time, associated with intermittent tinnitus. He has worked for 15 years with irregular use of personal protective equipment. The tympanic membranes are bilaterally intact and opaque. Prominent malleus handler. Audiometry and tympanometry were ordered to investigate the complaint, which resulted in a severe mixed loss on the right and moderate on the left. Exploratory tympanotomy in the right ear was indicated. During the surgery, the mobility of the ossicular chain was tested and the following were identified: fixed incus, malleus, and mobile stapes. The incus was disarticulated and a Y-shaped mold was made with a drill. The incus was coupled again between the malleus and stapes, forming an interposition. An incus autologous prosthesis was performed. After the surgery, there was an elevation in the audiometric curve and the patient referred hearing improvement. Conclusion: In this case of hypoacusis with an ill-defined etiology, exploratory tympanotomy allowed the diagnosis and treatment of the ossicular malformation using autologous material with a satisfactory result after surgery.
