Contribution of Mediastinoscopy in the Preoperative Staging of Non-Small Cell Lung Cancer.

Abstract
Aim: The Mediastinoscopy is an endoscopic examination of the mediastinum, to make biopsies particularly at the level of some lymph nodes areas. The absence in our center of mini invasive explorations make a diagnostic weapon of choice in the diagnosis of mediastinal lymph node invasion in lung cancer and especially in "Bulkys" and N3 to document the no indications for surgical resection of these cancers. Methods: Between 2009 and 2013, 204 patients have benefited from a mediastinoscopy, 21% were positive (N2 / N3). The surgical procedure was performed under general anesthesia through small incisions often axial, through which is introduced a mediastinoscope allowing visual exploration and biopsy of suspicious lymph nodes. Results: The average age is 58 years with a strong male predominance. We found 68% of adenocarcinoma. Patients were hospitalized for 24 hours and were released the next day after a radio control that was satisfactory. We have any deaths. Conclusion: Despite the evolution of minimally invasive explorations, mediastinoscopy remains an excellent alternative to exploratory thoracotomy when some lymph node stations are suspicious.
