Abstract
The article discusses research which aims to identify and compare factors to improving diabetes care in primary care practices in Pennsylvania. Positive deviance analysis suggests that the structural capabilities of primary care practices to buffer the stresses of change may be key facilitators of performance improvement in medical home transformations. Further, it indicates the importance of primary care practices for improving the likelihood of successful medical home transformation.
