Abstract
目的 探讨在颈丛阻滞麻醉下甲状腺手术中采用右美托咪定复合盐酸氢吗啡酮的临床麻醉效果.方法 回顾性分析2020年5月至2021年5月期间在我院进行甲状腺手术的80例患者的临床资料,全部患者均采用颈丛阻滞麻醉,根据不同的麻醉方案进行分组,采用芬太尼联合丙泊酚麻醉的患者纳入对照组(40例),采用盐酸氢吗啡酮联合右美托咪定麻醉的患者纳入观察组(40例),比较两组患者的麻醉起效时间、镇痛持续时间、术中麻醉效果和神经损伤情况.结果 与对照组相比,观察组的麻醉起效时间明显更短,镇痛持续时间明显更长(P<0.05);与对照组相比,观察组的术中可唤醒例数和颈丛穿刺时疼痛不明显例数明显更多,且神经损伤例数明显更少(P<0.05).结论 在颈丛阻滞麻醉下进行甲状腺手术,采用右美托咪定复合盐酸氢吗啡酮能有效获得更好的麻醉效果,延长患者的镇痛持续时间,降低神经损伤的发生率,值得临床进一步推广.
Objective To investigate the clinical anesthesia effect of dexmedetomidine combined with hydromorphone hydrochloride in thyroid surgery under cervical plexus block anesthesia.Methods The clinical data of 80 patients who underwent thyroid surgery in our hospital from May 2020 to May 2021 were retrospectively analyzed.All patients were treated with cervical plexus block anesthesia,and were divided into groups according to different anesthesia protocols.The patients anesthetized with propofol were included in the control group(40 cases),and the patients treated with hydromorphone hydrochloride combined with dexmedetomidine anesthesia were included in the observation group(40 cases).Time,intraoperative anesthesia effect and nerve damage.Results Compared with the control group,the onset time of anesthesia in the observation group was significantly shorter,and the duration of analgesia was significantly longer(P<0.05).The number of cases with no obvious pain was significantly more,and the number of cases of nerve injury was significantly less(P<0.05).Conclusion For thyroid surgery under cervical plexus block anesthesia,the use of dexmedetomidine combined with hydromorphone hydrochloride can effectively achieve better anesthesia effect,prolong the duration of analgesia,and reduce the incidence of nerve injury,which is worthy of further clinical treatment.promotion.
