Abstract
Home-based care of chronic patients cannot be efficiently provided away from shared care models. This paper introduces a middleware architecture to integrate technology-based home care services within shared care models. The architecture comprises a multi-access layer to provide seamless access to data and services; a computer-supported service for the care team collaboration; a wireless connectivity service to integrate telemonitoring sensors/devices and a security service. The middleware technology is developed and tested on three chronic care domains: chronic obstructive pulmonary disease, diabetes and HIV/AIDS care.
