Abstract
Reports of >60% recurrence of the saphenofemoral junction reflux after crossectomy due to incomplete surgical techniques and the formation of neoangiogenesis have led to a debate about the usefulness of this surgical procedure. In a treatment unit for venous diseases with an annual frequency of 700--750 inguinal crossectomies, this surgical procedure was enhanced with a focus on preventing such recurrences. Sewing up all side branches of the deep femoral vein in the groin with nonresorbable suture material, coagulating the endothelial surfaces of all central venous stumps, and reconstructing the deep fascia as an additional barrier added hardly any additional time during surgery and showed very good results regarding permanent correction of the saphenofemoral junction reflux after an average of 5 years. We believe that our surgical strategy--avoiding sloppy work and enhancing the surgical procedure to prevent neoangiogenesis--will help take the surgical treatment of varices into a new future.
