Abstract
Dear Editor, Severe exacerbations of acute intermittent porphyria (AIP) are treated with haem arginate, which inhibits -aminolaevulinic acid synthase in the liver, reducing the formation of neurotoxic precursors of haem which cause acute attacks [[1]]. Liver transplantation because of acute liver failure due to heme arginate overdose in a patient with acute intermittent porphyria. The proposed mechanism is saturation of the haem binding capacity of albumin and haemopexin with iron released from haem causing hepatic infarction and necrosis [[2]]. [Extracted from the article]
